The following are the international freight forwarding forms we use in transacting business.

Terms and conditions

This form contains the general contractual conditions provided by Smith Lewis & Staff to all its international freight forwarding services.

Letter of Authority

This is the sample letter that authorises SLS to act in behalf of its clients.

New Zealand Packing Declaration

This is a specific document used for sea freight forwarding services
to New Zealand.

LCL Packing Declaration

This document is specifically used for our LCL Freight Forwarding services.

FCL Packing Declaration

This document is specifically used for our FCL Freight Forwarding services.

Shippers Letter of Instruction

This form specifies details of the cargo to be handled by Smith Lewis.

Annual Packing Declaration

This form guarantees that the shipping container is free from material,
animal or plant origin and soil.

Customs Direct Debit [B320]

This Direct Debit Request authorises Smith Lewis to debit funds from
their Clients account.

EFT Client Authorisation [B322]

This form allows Smith Lewis to quote their client's nominated
bank account details.

Order to Pay Agent [B923]

This allows clients to authorise Smith Lewis to receive payment
in behalf of them.
